MedReferrals

We have created a wiki-type website which shows the location and services of many of the clinics in Chicago. This website allows you to search by what service a patient needs, where they are, what kind of insurance they have, and many other options. From there the website links to Google Maps so that the patient can get directions to the clinic by car or by public transportation. This is very important because this website is mainly geared towards the underinsured and homeless who may not have a car to get where they need to go.

We have a website. However, we want to make the website easier to use, to get physicians and patients to use it, and to expand our database. Right now we are calling clinics and getting their information one by one, but the eventual goal is to get clinics entering their own information so the website can sustain itself.

Many people need care, but do not know where to find it. This website would provide a way for these people to finally get care.

MedDiaries

A homeless man can come in to clinic having been to the ER the night before and have no idea of the treatment he recieved there. There are many drugs that interact or conditions that physicians need to be aware of to effectively treat a patient. Yet, for someone with no health record this is extremely difficult to keep track of, and at the end of the day the homeless just do not get the health care they need.

To rectify this, we have created a health care diary that the patient can take with them from doctor to doctor. That way they will have continuity in their health care, which is vital to their well being.

Right now there is nothing out there like the medical diaries we are trying to create.

We have created the model for the diary and are currently trying to produce it. From there we will go to the clinics that have high volumes of homeless patients and distribute the diaries. Then, we will keep track of how often they are being used at the clinics to guage effectiveness.
