MedReferrals is an Original Change Project initiative offering a free, comprehensive online health and social services database to provide greater access to quality care for the underserved. MedReferrals aims to bridge the disparity of unequal access to care by maximizing the value of already existing services in the community. By interacting directly with both healthcare providers and underserved communities, MedReferrals ensures an accurate, user-friendly resource. MedReferrals enables users to search for accessible services based on individualized needs such as languages spoken, type of insurance, and the cost of care. Customizable options allow healthcare providers to make effective referrals on a case-by-case basis. MedReferrals is committed to utilizing the most up-to-date resources to support our underserved communities.



MedDiaries

MedDiaries is an Original Change Project initiative to provide homeless individuals with a diary to maintain portable medical records that will serve as an additional communication tool to assist them as they navigate a potentially complex system of temporary support, housing, and clinical services. MedDiaries will provide a framework for more complete medical records and continuity of patient care that the Chicago homeless so greatly deserve and require. Stay tuned for more exciting updates and pictures of our progress.


MedFairs

MedFairs, an Original Change Project initiative to bring healthcare to places that need it most. MedFairs consists of health fairs at local food depositories and homeless shelters around the Chicagoland area. Volunteers provide basic health services including blood glucose and blood pressure screening, health education, government insurance information, as well as food preparation and distribution. In addition, Original Change Project is partnering with the following student organizations: Rush University Medical Center Chapter - American Medical Association, Young Educators, Student Interest Group in Neurology, Health Disparities Task Force, and Colleges Against Cancer. See below for a list of recently hosted health fairs:

  • January 20, 2010 – Humboldt Park Social Services Interim (6:45 – 7:45 pm)
  • February 8, 2010 – Humboldt Park Social Services Warming Center (6:00 – 8:00 pm)
  • February 10, 2010 – Breaking Bread (6:00 – 8:30pm)
  • February 22, 2010 – Lincoln Park Community Shelter (6:00 – 7:30pm)
  • February 27, 2010 – Hyde Park Food Pantry (10:00am – 1:00pm)
